2. Bus transportation*	If you are planning to drink at the event, we ask that you organize a car pool with a designated driver or take the bus which we have organized for the event. The bus is free for the first 40 to sign up. Otherwise the round-trip fee is 4,000 yen. Mark below if you would like to register for the bus service. YES NO
3. Comments/Questions:	Indicate here if you have any requests (example: playing partners). Note that in the spirit of the game, we will group two from each team (NA and EU) in the foursomes.

NOTE:

When you have submitted your application, you will be sent an email confirmation with the details for bank transfer of the registration fee of 3,000 yen.
If the registration fee is not paid by the deadline, your place in the event will be forfeited and offered to the next person at the top of the waiting list.
The remainder (21,000 yen) of the participation fee (+ what you eat and drink not included in the participation fee), must be paid directly to the club after the play.
